Please provide the project name exactly as it should appear on the quotation and on the report.
2. Complete Physical Address
Include the full physical address of the project site.
3. Demolition Plans and/or Representative Photos
Provide demolition plans (or the available plan) and/or representative project photos clearly indicating the components or areas to be included in our inspection.
Please include:
- At least two (2) representative photos of the structure and areas to be inspected
- A PDF or photo file of the demolition plan showing the structure and inspection areas
4. Contact Information
This refers to the person or company to whom the quote will be addressed and who will be responsible for payment.
Please provide:
- Full Name of the Person Requesting the Quote
- Company Name (if applicable)
- Email Address (to receive the quotation)
- Phone Number of the Requester
5. Escort / Access Coordinator Information
We require the name and phone number of the person who will grant access to the project site and coordinate the inspection visit.
Please include:
- Full Name of the escort or access coordinator
- Phone Number of the escort or access coordinator

- Inspection scheduling details

- Invoice for services
